TLC has disease management programs available to your patients with Asthma, Cardiovascular Disease, Diabetes, and High-Risk Pregnancies. TLC has developed these programs to assist your patients to better understand their condition, update them on new information and provide them with assistance from our staff. These programs are designed to reinforce your individual treatment plans. TLC members do not have to do anything to enroll in these programs. They are automatically enrolled when we identify them through authorizations or claims as a member with Asthma, Diabetes, Cardiovascular Disease, or High-Risk Pregnancy. There are also a disease specific case management programs for members with HIV/AIDS and Sickle Cell Disease. Each disease management program is guided by a steering committee made up of practicing providers including board certified specialists. These committees have developed disease specific work tools that you can use in your management of our TLC members. These population-based disease management programs provide the following services:
Support from our nurses and specialists to ensure that your patients understand their condition and how to best manage it. The healthcare staff will encourage compliance with office visits and your prescribed medical regimen.
Educational and informational materials that can assist your patients in understanding and managing the medications and activities that you prescribe.
Periodic newsletters to keep the patients informed of the latest information on their disease or condition.
Case management for patients with complex medical and psychosocial issues.
Access to 24 -hour telephonic support staffed by experienced registered nurses. These nurses provide answers to questions that are based on approved telephone protocols.
Information about upcoming community events such as health fairs.
We will inform you when your patients agree to participate in disease management and when they are discharged from our case management services. Membership in any of these disease management programs is voluntary. If at anytime members wish to stop participating in the program, they need only to call.
