Your Primary Care Provider is the main person you go to for your care.

In TLC, you will go to one main person for your health care.  He or she can be a doctor, a nurse practitioner, or a physician's assistant.  This person is called your Primary Care Provider or PCP.

The name of your PCP is listed on the front of your TLC identification card.

You can call your PCP anytime.  During after hours when you call, they will tell you how to reach your PCP.  If you cannot speak with someone after hours, please call TLC at 1-800-473-6523.

You should get to know your PCP.  Call to schedule a vist with your PCP as soon as you can.  This is important to do if you were receiving care or treatment from a different doctor.  We want to make sure that you keep receiving your care.  Even if you feel okay, you should call to get an appointment with your PCP.
